Inner Rhythms Acupuncture collects personal information from Clients, Business Partners, Contractors, Employees, persons enquiring about or purchasing a related service, private health funds, suppliers and visitors or Clients to our website and other social media.

We collect information that is necessary to be able to provide you with products or services. We require certain information in order to provide Chinese Medicine/Acupuncture services and in order to assess eligibility for such services.

In most instances, when you contact Inner Rhythms Acupuncture, we require, to know your name and contact information in order to provide you with the best and most appropriate information and/or for booking an Appointment. If the Health service provided to you does not require us to know this information, you may choose not to disclose it.

In relation to Client Intake forms, the types of personal information collected include your name, contact information, date of birth, email address, occupation, Medical Doctor, and Emergency contact If you have Children, or are in a Health Fund and which one. To further provide you with the most appropriate treatment, questions are asked regarding Family History and other in-depth questions on your medical history. These questions are required by us as the duty of care such as medical history and medications, other questions are used to refine and choose the correct Diagnostic pattern and what may seem irrelevant to your situation may be relevant for diagnostic purposes for a Chinese Medicine Practitioner. Your email address is used for Appointment confirmation, Courtesy reminders and Business information of importance. You have an opt-out marketing option on your Intake form. Please discuss this further with your practitioner if you have concerns on filling in your intake form. These forms and your medical history are required to be kept updated by you and you will be asked to complete a new form every 2 years if not notified beforehand.

How do we collect and hold personal information?

We collect, hold, use and disclose personal information so we can provide you with Chinese Medicine services. This may include:
•Checking your eligibility for Chinese Medicine service.
•Providing you with the Chinese Medicine service.

IT security measures will be reviewed regularly, maintained and implemented to ensure the most up to date measures are taken to protect the integrity of personal information.

We will engage the services of an accredited security organisation who is a member of the Australian Association of Information Destruction (NAID). No identifiable information will be disposed of with ordinary garbage disposal or recycling.
Destruction of personal information contained in electronic records will occur with the assistance of an authorised IT support organisation.
